accurate and complete. In order to enhance accurate testing and
subsequent reporting, (DA Form 1804) and laboratory sample
logbook must be completed correctly. See Figures 1 and 2. The
items that should be present and verified are as follows:
Product name and type of product
Laboratory sample number
Sample number assigned by customer
Specification number for the product being tested
The dates the tests were started and completed
The specification requirements are correctly entered.
Some laboratories have preprinted DA Forms 2077, which they use
with products that are routinely sampled.
For each product,
there is one DA Form 2077 which usually displays the product's
Deleted: '
name and specification. The remaining information is then added
at the testing facility or laboratory.
These preprinted forms
are used with products that are received from the same customer.
However, you should always perform the above checks and
comparisons to correct mistakes as well as to clarify
discrepancies. Use every opportunity to involve the technicians
in this comparison process. Remember that you are a trainer as
well as an evaluator.
