CONDITION:
You will be given completed DA Forms 1712-R, 1713-R, 1714-R, 1714-1-R and charts from
TB MED 577.
STANDARD:
Compare the information on the DA Form 1712-R to the requirement for source water
contained in TB MED 577, and eliminate from consideration sites that do not meet standards.
Compare the information on the DA Forms 1713-R, 1714-R, and 1714-1-R to standards
contained in TB MED 577. Certify potability, and authorize issue/distribution of water that
meets standard.

INTRODUCTION
In reviewing water analysis test findings from DA Forms 1712-R, 1713-R, 1714-R, and 1714-1R, it is first
necessary to compare the findings on the DA Form 1712-R to the minimum requirements for source water in TB
MED 577 and to eliminate from consideration the sites that do not meet the standard.
