If the commander wants operator or crew services scheduled,
put them in the Remarks Block.
floating equipment, or training aids and devices. Training aids and devices
are used only for training. Also, DO NOT use DD Form 314 for equipment when
automatic data processing (ADP) gives you printouts or automated forms with
all the data from DD Form 314.
(3) Number of DD Forms 314.
DD Forms 314 are tied to unit-level
services and their intervals. The number of forms you need varies based on
equipment and how and where your maintenance is pulled.
(4) Like Items.
Normally, one DD Form 314 covers one item of
equipment. But, several like items may be covered by one DD Form 314 if the
services are scheduled and pulled on the same date. Examples of like items
are small arms, telephone sets, trailers, and M11 decontamination apparatus.
When scheduling services on more than one item, put each item's serial
number in the Remarks block. Use the unit administration number for items
with no serial number.
You cannot combine like equipment or subsystems
(reportable under AR 700-138) on one DD Form 314.
(5) Schedule Services.
Use the front of DD Form 314 to schedule
services. Use the reverse side or another DD Form 314 to show NMCM or NMCS
times.
Schedule services at least one month or one service in advance,
whichever is greater. Count one month from the day you pull a service in
that month to the same date in the following month. For example, count one
month as the time from 12 March to 12 April. You may also figure a month
based on four weekly services.
Scheduling one month or one service in
advance is only a minimum.
You may schedule beyond that.
If the next
scheduled due date falls in a following year, put the date and miles or
hours due in the Remarks block until you start a new DD Form 314. Schedule
services in pencil.
Mark out weekends and holidays and schedule services
for the next working day.
(a) Symbols.
Show completed services by inking in the symbol or
symbols and miles or hours. Use the following symbols to show the type of
service scheduled:
