(8) When to Use DA Form 2404.
Personnel performing inspections,
maintenance services, diagnostic checks, technical evaluations, and PMCS use
DA Form 2404. You may use one DA Form 2404 or separate forms to inspect all
components or subsystems that make up one equipment system.
You may also
use one form to inspect several like items of equipment. For example, you
may use one DA Form 2404 to inspect 25 M16A1 rifles. Following are other
uses:

Temporary record of required and completed maintenance.
o
List of faults that the operators or crews cannot fix and the
faults corrected by replacing parts.
o
List of the faults that the unit maintenance people find and
the actions that they take to repair faults.
o
List of deficiencies
and
shortcomings
and
the
corrective
action taken.
o
List of faults that the support maintenance people find on

List of faults that the support maintenance people find during
the final inspection.
o
Collection of all the maintenance and services performed on
vehicles that are involved in a DA-approved shipment detail
card (SDC) plan.
o
Report of the battlefield damage repair or replacement actions
taken by unit-level maintenance.
