PART G - PREPARE MEMORANDUM FOR RECORD (MFR) FOR HIGHER
HEADQUARTERS
MFR for Higher Headquarters. Using data collected from DA Forms 1713-R, 1714-R, 1714-1-R,
2404, and 5456-R, you are to prepare a MFR for higher headquarters. This report may be required on a
daily, weekly, monthly, and/or quarterly basis as determined by the higher headquarters standing operating
procedures (SOP). This report should include information on the following:
The number of personnel and amount of equipment assigned to each water section, production and
storage capability.
Total hours of operation and the amount of water actually produced daily.
The amount of water actually issued or distributed daily.
The total amount of POL and chemicals used.
The units and number of personnel of the supported units.
The estimated gallons per man per day for the supported units.
The Surgeon General's or his representative's recommended chlorine residual at the point of distribution.
You will then need to attach copies of issue and distribution schedules, attach the appropriate cover
sheet for security of the document, and forward the report to officer in charge (OIC) for disposition of water
report to higher headquarters.
