-For PD 01-15 requests, follow up at the time of monthly reconciliation and when the estimated
shipping date has expired, after determination that valid supply status is unsatisfactory in terms of
estimated availability date, or 14 calendar days have expired since a request for cancellation was
submitted and no status has been received. Also, follow up for shipments with a final destination at
CONUS activities when 30 calendar days have passed since the data shipped shown on the shipment
status document; and the shipment has not been received.
Follow up for shipments with a final destination at Oversea Continental United States (OCONUS)
activities when 60 calendar days have passed since the date shipped shown on the shipment status
document and the shipment has not been received.
The following format is used when requesting a followup action: If a supply status card, (DA Form
2765-1) has been received, use the latest status card (AE1) and enter AF1 in document identifier block
(Figure 1). Enter the data from card columns 67, 68, and 69 of the original requisition in block 2 (if
zeros appear in these columns, leave block 2 blank). Next, circle blocks 1 and 2 using colored ink or
