d. After an individual from the unit has physically inventoried the subsistence and has
signed all the appropriate documents, you must adjust your inventory balances on hand. Select
the menu option that allows you to do this.
8. Processing Reports due to Loss of Subsistence. There are going to be times, and hopefully
not many, that your dining facility is going to have a loss of subsistence due either to
circumstances beyond your control, or by negligence. You may be the AFMIS user who enters
the data that relieves the Senior Food Operations NCO of accountability and responsibility for
those items. You must follow correct procedures and be accurate in your entries.
a. When you have a loss of subsistence due to negligence or another cause outlined in AR
735-5, you should recover those losses by processing a Statement of Charges (SOC). Your
AFMIS system allows you to process this document DD Form 362 (Statement of Charges for
Government Property Lost, Damaged, or Destroyed).
b. Once the DD Form 362 is started, the unit responsible for the operation of your dining
facility assigns the document control number. The responsible commander and the individual
accepting responsibility sign the statement. The DD Form 362 is forwarded to the finance and
accounting office (FAO) for action.
c. You can release your AFMIS statement to TISA after the unit assigns a document
control number. TISA uses the statement to credit your dining facility account.
d. You must send copies of the DD Form 362 and your AFMIS Statement of Charges
Report to TISA and the IFA. TISA must have these reports to process your SOC.
9. Issue Checklist Inquiry Subprocess. Use this subprocess of AMFIS to review the status of
production and issue processes by meal date. You can review meals selected, projected and
adjusted headcounts, and shopping list status information.
