c. The headcounter is responsible for accurately accounting for each individual who is
admitted to the dining facility for a meal. He or she must be able to identify all personnel who
enter the dining facility and verify their meal entitlement.
(1) The headcounter will check each individual's DD Form 714 (Armed Forces
Identification Card), 714-E, DD Form 2A, or travel orders for verification of entitlements.
(2) The headcounter will not be required to pull any additional duties such as checking
attire and maintaining order.
16. DA Form 3032. The DA Form 3032 is a multipurpose document used to account for meals
served and the basic document to collect signatures and account for meals served to enlisted
personnel authorized SIK.
17. DA Form 3033. The DA Form 3033 (Headcount Record) is used to summarize the data
contained on the DD Form 1544 and DA Form 3032. The FOS or contractor will extract from
these forms the total number subsisted in each category. All headcount data will be attached to
the DA Form 3033 prior to its filing. To correcting mistakes on DA Form 3033, the original
figure will be lined through (original entry must be readable) and the correct figure inserted. The
individual making the change will initial the correction. An explanation for the correction will be
made in the "remarks" block and supporting documents attached to the DA Form 3033.
18. DA Form 2970 (Subsistence Report and Field Ration Request). This form serves as serves
as a ration request and as a feeder report for units in garrison operating under the Army Ration
Credit System (ARCS). It also provides the means to report the number of authorized personnel
subsisted in active Army, Army National Guard (ARNG), and United States Army Reserve
(USAR) dining facilities operating under ARCS.
a. Unit Requirements. All garrison units are required to submit assigned and attached SIK
strength data to the Personnel and Administration Center (PAC) where the information will be
consolidated and submitted to the dining facility on a daily basis. The data is required by the
dining facility for menu planning and subsequent submission to the troop issue subsistence
officer (TISO) for preparation of DA Form 2969-R.
b. Dining Facility requirements. Active Army dining facilities drawing rations from troop
issued subsistence Activity (TISA) will submit consolidated headcount data. Dining facilities
will also report headcount data from the issued operational rations.
19. DD Form 1544 (Cash Meal Payment Sheet). The DD Form 1544 is used to record the cash
meal payment for government-furnished meals sold to authorized personnel. These sheets are
issued in a prenumbered covered book consisting of 50 individual sheets. It contains instructions
for using the book, transfer control and receipts, and a register for the issue of individual sheets.
More specific information about procedures for the use of DD Form 1544 is provided below and
in the following paragraphs.
