1-3. Forms
Ensure that appropriate personnel prepare all dining facility administration forms
Preparation
according to particular requirements. Specific guidance for these provisions
follows:
Complete the forms with a pen or a typewriter that dispenses blue, black, or
blue-black ink. DO NOT use erasable pens.
Ensure that whoever makes changes draws a single line through all posted
changes and initial them. DO NOT use correction fluid, correction tape,
markers, or other blocking-out devices.
Reproduce these forms locally on 8 1/2-by 11-inch paper if the form numbers
end with an "R."

Mark DA Form 3034 (Production Schedule) with a red-ink marking device to
highlight and record leftover food items that are authorized for retention. Do
this according to TB MED 530 (Food Service Sanitation).
The DA Form 3980-R is maintained at the dining facility for all full-time
dining facility accounts operating under ARCS. All entries posted to the
record may be in pencil except the initial posting, final figures, and
verification by the troop issue subsistence officer (TISO).
Mark through inapplicable words. Enter the applicable RC designation. For
example, mark out "Active Army" and enter "USAR" or "ARNG." Doing so
changes the DA Forms prescribed for use in this subcourse.
