Ensure that the FOS enters the full unit designation, the installation, and the zip

code. For example, "2/327th Air Assault, Ft Campbell. KY 37040" is entered in
the sample DA Form 3032 that is shown in Figure 2-1.
Make sure that the FOS enters the day, the month, and the year. For example, the
Date
date entered in the sample DA Form 3032 that is shown in Figure 2-1 is "AUG
86."
Make sure that the FOS enters an "X" in the appropriate box. For example, the
Meal
"Lunch" box is checked in the sample DA Form 3032 that is shown in Figure 2-1.
Ensure that the FOS enters the abbreviated service component for the
Service
personnel who will sign the form. The abbreviated service components appear on
component
the bottom of the back of the form. For example, the sample DA Form 3032 that
is shown in Figure 2-1 shows the entry "USA" (for AA).
Ensure that the FOS--
Diner category
Enters an "X" in the appropriate box.
Uses a separate DA Form 3032 for each category or group within a category
each day.
Uses the category "other" for categories that are not shown. Examples of
"other" categories are the Korean Augmentation to the United STATES
(KATUSA) and the Civilian Labor Group (CLG).
The diner enters his first name, his middle initial, and his last name as he

normally signs them. He enters this data before he consumes the meal. Refer to
Figure 2-1 for examples.
