Ensure that the headcounter-number--
Meal card
Enters the meal card number as it appears on the DD Form 714.
Enters the last four digits of the SSN as it appears on an ARNG or a USAR
member's DD Form 2A. This is for trainees at BCT or AIT.
Enters the individual's orders number. The orders number must have been
validated in accordance with AR 600-38. The meal card number or the SSN
appears upside down when it is annotated properly. The sample DA Form
3032 that is shown in Figure 2-1 shows examples of these entries.
Lines out all unused lines at the close of the meal by drawing a straight line
from the upper left to the lower right for all lines in each column. The sample
DA Form 3032 that is shown in Figure 2-1 shows examples of how to line out
unused lines.
Uses a new DA Form 3032 for late meals served.
To clarify any headcount data, ensure that the headcounter enters remarks on each

DA Form 3032 that appears unclear and that will provide an audit trail. Ensure
that when the headcounter is not present. a member of the dining facility staff
performs this function. Figure 2-2, Page 2-13 shows an example of an entry to
the "Remarks" block of this form.

Headcounter enters his grade at the conclusion of each scheduled meal
service.
FOS signs the DA Form 3032 after he reviews it, but before he consolidates
the headcounter data on DA Form 3033.
